WBBC Financial Assistance Program Eligibility

Individuals must meet all 5 of the following criteria in order to qualify for the Well Beyond Breast Cancer Financial Assistance Program.

Diagnosis. Individual has been diagnosed with any stage of breast cancer or a high-risk breast cancer gene mutation. Verified by providing an official biopsy report or genetic test result.

Treatments. Individual has undergone treatments (surgery, chemotherapy, radiation therapy, and/or other) for breast cancer or a high-risk breast cancer gene mutation. Verified by providing a one-page treatment plan from your physician, nurse navigator, or hospital social worker.

Impairments. Individual is experiencing cancer-related side effects which are impacting their quality of life and well-being. Verified by completing the DASH and FACT-B questionnaires (provided to you in application packet).

Geographical Region. For the moment, we are only serving individuals residing near New York City/Westchester (New York), Fort Worth (Texas), Oakland (California), and Minneapolis (Minnesota). Verified by providing your address on the application form.

Income Level. Individuals’ income must not exceed 500% of the 2024 Federal Poverty Level (FPL) guidelines (see below for guidelines). Verified by providing your most recent 1040 tax return.

Financial Assistance Program FAQs

Can I apply if I have private insurance?

All private insurance plans are different, thus this depends on your exact insurance coverage. To find out if you qualify for our Financial Assistance Program based on your insurance status, please fill out an application contact form (found here).

Please note that preference is given to applicants who are uninsured, have Medicaid insurance, or who don’t have out of network insurance benefits. At this moment, we do not accept patients who have Medicare insurance.

Will preference be given to applicants who are currently undergoing treatments?

No preference is given regarding treatment status. Applicants can apply during their cancer/prophylactic treatments, when in remission, and even years after the completion of their treatments.

I am interested in applying, how do I fill out an application?

Please start by filling out our application contact form (found here). Once we receive your form, our Financial Aid Advisory Committee will email you an official application package, to be completed at your discretion.

How do I submit my completed application package?

Once you have completed your application package, please submit it for our review by emailing info@wellbeyondbreastcancer.org.

We do not accept applications sent by mail nor fax.

Once I have submitted my completed application package, when should I expect to hear back?

Once we receive your entire application package, you will be emailed a confirmation of receipt.

Applications for the WBBC Financial Assistance Program are reviewed by our Financial Aid Advisory Committee twice monthly. So on average, you can expect to hear back from us within two-three weeks of receiving your completed application package.

We will contact you by email regarding your application status. Please be sure to check your junk/spam folder for emails from Well Beyond Breast Cancer, as email is our primary form of communication.

What happens if I am approved?

You will be emailed notifying you of the status of your application. In the same email, you will also be provided with the contact details (phone number, address, email, and name of the treating therapist) for the physical/occupational therapy clinic where you will go for breast cancer rehabilitation and/or lymphedema therapy in your community.

It is then your responsibility to schedule your first appointment with the designated physical or occupational therapist. If no appointment is scheduled within 30 days of receiving approval for our Financial Assistance Program, this can result in your funding being revoked.

If approved, how many appointments of breast cancer rehabilitation/lymphedema therapy will WBBC pay for?

You will be approved for 12-16 one-hour appointments of breast cancer rehabilitation/lymphedema therapy, based on individual circumstances.

How will my appointments be paid for?

Your treating physical/occupational therapy practice will bill Well Beyond Breast Cancer and Well Beyond Breast Cancer will directly pay for your appointments of breast cancer rehabilitation and/or lymphedema therapy. No money will be sent to the individual approved for our Financial Assistance Program. This avoids any additional paperwork for you, so that you can focus on your recovery.

If approved, can I use the funding to cover other healthcare related expenses?

No. The funding provided by the WBBC Financial Assistance Program can only be used towards appointments of physical or occupational therapy with your designated therapist specially trained in breast cancer rehabilitation and/or lymphedema therapy.

Can men who have been diagnosed with breast cancer or a high-risk breast cancer gene mutation apply?

Yes, we are gender inclusive.
